Bill of lading

  • This form should be filled out by the shipper & signed by the driver
  • Please include a description of the type(s) of packaging used and exact number of pieces that must be physically moved for the total piece count

Shipping Label

  • This form can be used as an optional shipping label

Textiles declaration

  • This form must be filled out when importing textiles into the US or when textiles are returning to the US after a show
  • Carpet padding does not need to be declared on the form, only carpet & fabrics
  • Only one form is needed for each textile being shipped

Commercial invoice

  • This form must be filled out when shipping anything outside the United States
  • You must have the shippers EIN or Federal Tax ID in order to properly complete this form

FCC 740 – Radio Frequency Form

  • These forms must be filled out when importing electronic goods into the US or when these type of goods are returning to the US after a show
  • Monitors, hard drives, mice, keyboards, and electronic equipment that is being shipped MUST have a 740 US customs form and a 2877 FDA form filled out for each and every device
  • The information you need will typically be found on the back or the bottom of the equipment and will stipulate the serial number, model number and where manufactured (country). You may need to look on the web for the actual address of the manufacturer
