Bill of lading

This form should be filled out by the shipper & signed by the driver
Please include a description of the type(s) of packaging used and exact number of pieces that must be physically moved for the total piece count

This form must be filled out when importing textiles into the US or when textiles are returning to the US after a show
Carpet padding does not need to be declared on the form, only carpet & fabrics
Only one form is needed for each textile being shipped

This form must be filled out when shipping anything outside the United States
You must have the shippers EIN or Federal Tax ID in order to properly complete this form

These forms must be filled out when importing electronic goods into the US or when these type of goods are returning to the US after a show
Monitors, hard drives, mice, keyboards, and electronic equipment that is being shipped MUST have a 740 US customs form and a 2877 FDA form filled out for each and every device
